1 . What is the UNIVERGE® Applications and Terminals Manufacturer’s Rebate Program?
It is a rebate program offered directly to NEC end-users to demonstrate our appreciation of your purchase of an NEC UNIVERGE SV8300 or SV8500 Communications Servers, and the UNIVERGE Desktop IP or Digital Terminals and advanced applications that are supported on it. 2. How do I know if I am eligible for the rebate?
To be eligible to receive a Rebate:
- Order a new UNIVERGE SV8300 or SV8500 and Qualifying Products (UNIVERGE Desktop IP or Digital Terminals and advanced applications listed on the Rebate Form or the Rebate Calculator) between August 18, 2008 and March 31, 2009 from NEC or any authorized NEC Associate or Reseller.
- Be a U.S. resident
- Order made and delivered within the U.S.
3. Where do I find the Rebate details and forms I will need?
All of the forms, details, and terms and conditions are on the NEC web promotion’s web site: http://www.necunified.com/promotions
4. What do I need to do to receive the rebate?
Download and fill out all of the Rebate form accurately and legibly. (Incomplete, invalid, illegible, inaccurate, misdirected, fraudulent or late submissions are ineligible for the Rebate and WILL NOT be processed.)
Also, please provide a copy of your invoice(s) listing the new UNIVERGE SV8300 or SV8500 and Qualifying Products ordered from NEC or any authorized NEC Associate or Reseller and an executed W9 form.
5. Where do I find the telephone serial number for the Rebate form?

6. What other documentation will I need besides the Rebate form?
- A copy of your invoice(s) listing the new UNIVERGE SV8300 or SV8500 and Qualifying Products ordered from NEC or any authorized NEC Associate or Reseller
- An executed W9 form
7. What is the deadline for submitting the Rebate form and other documentation?
All submissions must be received by NEC no later than 5:00 p.m. (Central) on May 29, 2009. All submissions received after this date and time will not be accepted.
8. Where do I send my Rebate forms and documentation?
Fax the completed Rebate form to (214) 262-2582 Attention Rebate Dept. or email completed scanned form to rebate@necunified.com.
9. Once my Rebate forms and documentation are submitted, how long before I can expect to receive my Rebate check?
Allow up to ten (10) weeks for mailing of Rebate after NEC is in receipt of a complete and valid submission.
10. Am I allowed to substitute different products?
No product substitutions, deletions, or additions or gifts are allowed in lieu of Rebate.
11. Can the Rebate check be paid to an individual?
No. NEC will only pay Rebate checks to companies who complete a W-9 and submit it along with their completed rebate form and copy of invoice.
12. Can this Rebate offer be combined with any other NEC promotion?
No, this Rebate offer cannot be combined with any other offer unless specifically agreed to by NEC in writing.
13. Can I assign or transfer this Rebate offer to someone else?
Your rights to this Rebate offer cannot be assigned or transferred and this offer is void where restricted or prohibited by law.
14. What happens to the documentation that I send to NEC?
All materials and documentation submitted become the property of NEC and shall not be returned.
