Rebate Form Instructions

Turn over digital or IP telephone and write down the serial number.  See example circled to the right. Include a copy of the invoice(s) listing the new UNIVERGE SV8300 and Qualifying Products ordered from an authorized NEC Unified Solutions Dealer or Associate between August 18, 2008 and 5 p.m. (Central) March 31, 2009. An executed W9 form must also accompany this form. Complete all sections of this form and fax to (214) 262-2582, attention Rebate Department on or before 5 p.m. Central on May 29, 2009.

NEC Unified Solutions, Inc. (“NEC”) UNIVERGE Rebate Offer ("Rebate Offer") Terms and Conditions

1. ABSOLUTELY NO SUBMISSIONS WILL BE ACCEPTED AFTER 5pm (Central) on May 29, 2009.
2. To be eligible to receive a Rebate, order a new UNIVERGE SV8300 or SV8500 and Qualifying Products (as defined on the “What solutions are eligible” page) between August 18, 2008 and March 31, 2009 from NEC or any authorized NEC Associate or Reseller.
3. Provide all requested information accurately and legibly.
4. Fax the completed Rebate form to (214) 262-2582 Attention Rebate Dept. or e-mail completed scanned form to rebate@necunified.com.   Submissions must be received by NEC on or before 5 p.m. (Central) on May 29, 2009 to be eligible for this Rebate.
5. Incomplete, invalid, illegible, inaccurate, misdirected, fraudulent or late submissions are ineligible for the Rebate and WILL NOT be processed.
6. Non-U.S. residents are ineligible for this Rebate Offer.  
7. Orders made or delivered outside the U.S. are ineligible for this Rebate Offer.
8. Allow up to ten (10) weeks for mailing of Rebate after NEC is in receipt of a complete and valid submission.
9. Rebate shall be paid in U.S. dollars and failure to cash a Rebate check within one hundred eighty (180) days of issuance shall void the Rebate and NEC shall have no obligation to pay the Rebate.
10. No liability will be incurred for any late, lost or misdirect submissions, errors, technical difficulties of any kind (including, but not limited to the inability to access: the facsimile number above; the NEC website http://www.necunified.com/promotions/ or the Rebate link thereon; and/or the email address rebate@necunified.com), omissions, processing delays, performance or the failure to perform hereunder or any claims or losses arising hereunder.
11. No product substitutions, deletions or additions or gifts in lieu of Rebate will be allowed.
12. This Rebate Offer cannot be combined with any other NEC offer unless specifically agreed to by NEC in writing.
13. Your rights to this Rebate Offer cannot be assigned or transferred and this offer is void where restricted or prohibited by law.
14. NEC reserves the right, at its sole discretion, to disqualify anyone who tampers with the Rebate fulfillment process.
15. All materials submitted become property of NEC and shall not be returned.
16. Rebate is offered only in the United States and is governed by the laws of the State of Texas , and all claims must be resolved in the Federal or State Courts located in Dallas , Texas , without regard to conflicts of laws principles.
